Pseudochylothorax

 

Chylothorax is the accumulation of chyle (lymphatic fluid rich in triglycerides) in the pleural cavity due to damage or obstruction of the thoracic duct

Tuberculosis
 

Modern genetic studies of the TB bacterium show that:

  • Mycobacterium tuberculosis is very old 
  •  (Approximately 9000 Years Old)
  • It evolved along with humans 

Evidence of TB has been found in:

  • Egyptian mummies (~3000 BCE)
  • Ancient Indian and Chinese texts
  • Skeletal remains showing spinal TB (Pott disease) 

Hippocrates (460–370 BCE) described a fatal wasting disease affecting young adults, likely pulmonary TB.

Pseudochylothorax has a strong historical association with tuberculosis, because before the antibiotic era TB was the most common cause of chronic pleural disease, which is the main setting in which pseudochylothorax develops. 
  

Mechanism in TB:

  1. Chronic TB pleuritis
  2. Fibrosis and pleural thickening
  3. Long-standing trapped effusion
  4. Cell breakdown → cholesterol release
  5. Cholesterol-rich milky fluid
