Arias Stella Reaction

 

Dr. Jorge Arias-Stella, a Peruvian pathologist, who first described the phenomenon in the 1950s in relation to abnormal endometrial changes during pregnancy.




The Arias-Stella reaction is a benign, non-neoplastic change seen in the endometrial glands of the uterus. It is a hormone-induced cellular response, primarily due to elevated progesterone levels, typically during pregnancy or with hormonal stimulation (e.g., from oral contraceptives or certain tumors).

The endometrial glands show:

  • Hyperchromatic, enlarged nuclei

  • Nuclear atypia (can resemble malignancy)

  • Irregular nuclear contours

  • Increased mitotic activity is absent

  • Cytoplasmic vacuolization

  • Decidualized stroma may be present
